    1. Referral Membership allows you to display your contact information and displayed on our website. Our patients are always asking if we have another doctor of CPK/BSI where their family members live...you can receive referrals for only $50 a year.
    2. Educational Membership allows you to receive educational seminars and the annual conference at discounted rate, plus access code for website educational material, and invitations to periodic webinars for only $200 a year.
    3. Star Membership is to register for both and receive a complementary educational DVD.
